The first time I heard of neuroplastic pain was from Alan Gordon's book 'The Way Out'. I was desperately searching for answers about my son's pain. My son and I listened to it together on Audible whilst he was in hospital, on an epidural to help his excruciating pain.
The idea of the brain learning pain and pain being linked to the nervous system was new to me. It planted a seed. But it was months later, after exhausting other options, that I sought out a PRT coach for him. What followed changed everything. The short answer: Yes.
PRT can help with pain associated with endometriosis, sciatica, disc bulges, some arthritic conditions, post-surgery, nerve pain and many other underlying conditions.
Neuroplastic pain is influenced by the nervous system. When suffering from chronic pain, this naturally causes stress. Stress causes the nervous system to be heightened, which can then amplify pain. That's one component.
The other component is that the brain learns pain. With chronic pain, the brain can learn patterns of pain, and the neural pathways involved in pain can become stronger the more they are used. This can make pain more likely to continue, even when there is less danger or tissue damage than there was originally.
My son had nerve damage in his entire left leg, so he had a physical cause to his pain. This is why it took me months to find him a PRT coach even after reading Alan Gordon's book 'The Way Out'. It's the question people want to ask, but they're scared to ask. And I understand why - when you've been in pain for months or years, hope can feel dangerous. You don't want to get your hopes up again. So I'll be honest with you: the answer is: it depends. But in a good way! If your pain has no clear physical cause If your scans keep coming back normal, if doctors can't find a structural reason for your pain, then, yes, full recovery is absolutely possible. This kind of pain is neuroplastic. Your brain has learnt pain. The neural pathways involved in producing pain signals have become well-worn through repetition, not because something is broken in your body. And because the brain learnt it, the brain can unlearn it. Pain Reprocessing Therapy works directly with this. Part 1 of 4 on central sensitisation and chronic pain series. If you've been living with persistent pain, you may have come across the term central sensitisation (or pain sensitivity). It's a concept that's transforming how we understand and treat chronic pain, and it may hold the key to your recovery. Your nervous system has a volume knob. Think of your nervous system like a sound system. Normally, the volume is calibrated to match the signal - a gentle touch feels gentle, and a serious injury feels painful. That calibration is what keeps you safe. Central sensitisation is when that volume knob gets turned up too high and gets stuck there.In this state, your nervous system becomes hypersensitive. It begins to amplify pain signals, sometimes generating pain in the absence of any real tissue damage, or sometimes amplifying pain from actual tissue damage. Things that should feel neutral (a light breeze on your skin, a gentle touch, normal movement), can start to feel threatening or painful. Your brain isn't broken, and you're not imagining it! Why Does Central Sensitisation Happen? Amu Satchi, Pain Reprocessing Therapy Practitioner, Sydney, Australia

Part 2 of 4 on central sensitisation and chronic pain series. The nervous system is remarkably adaptable, a quality called neuroplasticity. This is generally a good thing. It's how we learn, form memories, and recover from injury. But neuroplasticity works in both directions. When pain signals fire repeatedly over time (whether due to injury, illness, stress, or emotional trauma) ,the pathways that carry those signals can become more and more efficient. Like a well-worn path through long grass - the more it's used, the easier it becomes to travel. The nervous system essentially learns to be in pain. Several factors can contribute to this process: ✦ Prolonged or untreated pain. When pain persists, the nervous system can undergo physical changes. Neurons in the spinal cord and brain become more excitable, and the threshold for firing pain signals lowers. Less and less stimulus is needed to trigger a pain response. ✦ Stress and the threat response. Your brain's primary job is to keep you safe. When you're under chronic stress - whether from physical danger, workplace pressure, relationship difficulties, or unresolved trauma - your nervous system stays in a state of vigilance. Pain is the nervous system's alarm system, and when the alarm system is primed, it triggers more easily. ✦ Learned neural pathways. Research in neuroscience has shown that the brain can learn pain the same way it learns any other pattern. Pain that began as a signal of tissue damage can become a habit of the nervous system, continuing long after the original cause has resolved. ✦ Psychological and emotional factors. Fear, anxiety, and low mood are not simply reactions to pain, they actively amplify it. When the brain perceives pain as threatening or inescapable, it increases the pain signal. Part 3 of 4 on central sensitisation and chronic pain series. Central sensitisation is can play a significant role in a wide range of chronic pain conditions. These include:◆ Fibromyalgia - Widespread muscle and joint pain, fatigue, and heightened sensitivity to pressure. Fibromyalgia is one of the clearest examples of a condition driven by central sensitisation, with no identifiable tissue damage underlying the pain.◆ Chronic low back pain - While back pain often begins with a structural trigger (a disc bulge, muscle strain, or injury), in many cases the pain persists and spreads well beyond what the tissue damage can explain. Central sensitisation is increasingly recognised as the driver of this ongoing pain.◆ Irritable Bowel Syndrome (IBS) - The gut is richly innervated, and central sensitisation can cause the nervous system to amplify normal digestive sensations into significant pain and discomfort.◆ Migraine and tension-type headaches - Recurrent headaches can involve sensitised pain pathways in the brain, which lower the threshold for headache episodes and increase their intensity.◆ Temporomandibular joint disorders (TMJ) - Jaw pain and facial pain conditions frequently involve a central sensitisation component, particularly when pain spreads beyond the joint itself.◆ Complex Regional Pain Syndrome (CRPS) - A condition characterised by severe, burning pain, often in a limb, that is disproportionate to any original injury. CRPS is widely considered a central sensitisation condition and responds well to approaches that target the nervous system directly.◆ Chronic pelvic pain - Including conditions like vulvodynia and interstitial cystitis, where the pelvic region becomes a site of ongoing hypersensitivity in the absence of ongoing tissue damage.◆ Post-surgical pain - Some people experience significant pain long after surgery, even when healing is complete. The nervous system can maintain a pain response that was appropriate during recovery, but has continued past its useful purpose.◆ Sciatica and nerve pain - While sciatica often has a structural origin (such as a disc pressing on a nerve), the persistence and spread of that pain is frequently amplified by central sensitisation. The neural pathways involved in radiating leg pain can become sensitised, generating pain even when the original compression has resolved.◆ Endometriosis-related pain - Endometriosis involves real tissue changes, but the severity and persistence of pain in endometriosis is not always proportional to the extent of those changes. Central sensitisation helps explain why some people with minimal endometriosis experience severe pain, and why pain can persist even after treatment.◆ Rheumatoid arthritis and osteoarthritis - While these conditions involve genuine joint changes, research shows that many people with arthritis also have a central sensitisation component that significantly amplifies their pain experience beyond what the joint damage alone would produce. While these conditions may look very different on the surface, they share one important feature: an overprotective nervous system that has become stuck in high alert. The good news is that when we understand how this process works, we can begin to retrain the brain and reduce pain. What Causes Pain Flare-Ups? And How Do I Stop Being Scared of Them?

If you live with chronic pain, you know the feeling. Things have been going okay, and then, out of nowhere, the pain spikes again. Your mind jumps straight to the worst-case scenario: something's wrong, I've damaged myself, I'm back to square one.
Here's the reassuring truth: a flare-up is not usually a sign of new damage. It's your nervous system reacting, not your body breaking down. Understanding this is often the first step to taking away a flare-up's power. And it's exactly the kind of work we do together in Pain Reprocessing Therapy (PRT).

What causes a pain flare-up?

A flare-up happens when your nervous system's "volume knob" turns up again, amplifying pain signals - usually in response to a trigger like stress, overexertion, poor sleep, or even an emotional event. It's rarely a sign of new tissue damage. It's your protective system being extra vigilant, not your body being re-injured.

Why do flare-ups happen even when nothing has changed physically?

This is the part that confuses people most. If your pain is neuroplastic, or has a strong neuroplastic component, your nervous system has learned pain pathways over time. Those pathways can be triggered again by all sorts of things that have nothing to do with physical damage:✦ Stress and overwhelm: a big week at work, an argument, financial pressure ✦ Poor or disrupted sleep: sleep is one of the nervous system's main recovery windows ✦ Overexertion or a change in activity: doing more than your body is used to ✦ Emotional triggers: grief, conflict, anxiety, even excitement ✦ Illness or fatigue: being run-down lowers your threshold ✦ Weather changes: genuinely reported by many people, likely linked to subtle shifts in nervous system sensitivity None of these mean something is newly wrong with your body. They mean your nervous system has been nudged back into a more protective, high-alert state.

Why does being scared of a flare-up make it worse?

Fear itself is fuel for the pain cycle. When a flare-up happens and you respond with panic, "it's happening again, I'm never going to get better", your nervous system reads that fear as further confirmation of danger. It turns the volume up even more. You end up in a loop: pain causes fear, fear amplifies pain, and the flare-up lasts longer and feels more intense than it needs to.This is why simply gritting your teeth and "pushing through" isn't the answer, but neither is retreating completely. The goal is to change your relationship to the flare-up.

How do I respond to a flare-up without fear?

A few things can help in the moment:1. Remind yourself of the facts. A flare-up is your nervous system's alarm system, not new damage. Repeating this to yourself, calmly and often, starts to retrain the response over time.2. Avoid catastrophic thinking. Thoughts like "this will never end" or "I've undone all my progress" will fuel the fire. Try to notice these thoughts without believing them completely.3. Stay gently engaged with normal activity. Complete rest can reinforce the idea that your body is fragile. Where possible, keep moving in ways that feel manageable.4. Send your body safety signals. Slow breathing, a warm shower, reassuring self-talk, or simply lying down and letting your nervous system settle can all help turn the volume back down.5. Expect flare-ups as part of the process, not a sign of failure. Recovery from neuroplastic pain is rarely a straight line. Flare-ups often become shorter and less intense over time as your nervous system learns it's safe. The bottom lineA flare-up feels alarming, but it's very rarely a sign that something is newly wrong. It's your nervous system reacting to a trigger - and the more you can respond with calm reassurance instead of fear, the faster it tends to settle. Over time, with the right tools, flare-ups become less frequent, less intense, and far less frightening.
